I'm embarred to tell my family...?

WARNING EXPLICIT CONTENT! Hi, im 14 and unfortunately I got a skin rash developed on my bladder/gall like a year ago. My bladder starts itching at usually night time when I'm in bed and I've been trying not to itch it so maybe it will get ok but its impossible. After that my skin gets a rash and sometimes wet. There is no pain when I pee or anything its just scratchy at night. I don't know even know where the clinic is so I would have told my doctor(I'm only 14). I can't tell my family about it because I'm embarred. Sorry if I disgusted you but this is the condition I live with everyday. What should I do?
